Those prices are hilarious. Rapid charge I'd be prepared to pay for, I'll never use the others.
In Denmark we charge by kW which seems much more fair to me. 60pence for 1kW with a free subscription. 37pence for 1kW if you pay a 11£ pr. month subscription. The price is same no matter if use a 50kW or 3,7kW charger.
Pennies per kWh, with a minimum of $5 should be the standard.
